Essential Functions

Primary Responsibilities for all Units Assigned:

Provides clinical services and care for patients.

1. Answers multi-line telephone, routing calls and providing messages.
2. Documents Daily Census updates and changes.
3. Printing patient education materials, pertinent ancillary reports for patients/Nursing Staff during patient stay and at discharge.
4. Schedule follow up appointments at discharge for patients.
5. Checking orders input and assisting with order entries per protocols.
6. Registering patients.
7. Assisting with patient transfers.
8. Initiates, assembles, and maintains charts for all patients on the unit. Makes new charts as needed.
9. Checks all charts when physician has completed them to obtain STAT and discharge order.
10. Notifies Housekeeping of room numbers after discharge in a timely manner.
11. Informs nursing staff of pertinent information regarding patient care and procedures from ancillary departments.
12. Reports patient diet type, food allergies, and preferences to the Dietary Department. 
13. Communicate patient status, pending orders and information during transfers, at exchange of shift, and as needed.
14. Notifies Central Supply of needed supplies for the unit.
15. Assisting the Nuse Aide when needed.
16. Enters patient service charges, assists with contract agreements, and payment collections.
17. Other duties as assigned.

Job Qualifications

Education

• One (1) year of nurse aide experience in a clinical or long term care setting preferred.

Essential Functions

Primary Responsibilities

1. Assist with carrying out the Interdisciplinary Plan of Care for residents.
2. Assists residents with activities of daily living according to their Care Plans and MHCC Policies and Procedures.
3. Documents residents’ care according to established MHCC P&P’s.
4. Keeps the Charge Nurse informed regarding resident change of condition or other observations such as symptoms, reactions, or other changes in patient status.

Administers direct bedside nursing care under the supervision of the Charge Nurse which may include:

1. Assists residents with fingernail care as scheduled and needed.
2. Transfers residents from bed, to chair, to commode, to gurneys using gait belts and lifts as appropriate.
3. Assists residents in bathing to include bedside sponge baths and hall showers regularly or as needed.
4. Assists residents in oral care to include brushing teeth, brushing, soaking, and adhering dentures, and clearing oral cavity of debris after meals prior to residents resting.
5. Assisting residents in eating by feeding, or monitoring proper airway protection methods prescribed.
6. Maintains residents dignity and privacy at all times.
7. Assists residents in restroom activities as needed to include changing soiled adult protection diapers, clothing and bedding, assisting to bedside commode, room commodes as needed and providing optimal perineal care as needed according to the residents’ care plans to reduce potential for infection taking care to preserve resident privacy and dignity.
8. Ensures residents are in safe environment to include lowering beds and clearing furniture path as needed.

Provides assistance and performs support duties as needed.

1. Prepares, maintains, and submits records and reports required by the Charge Nurse, Director of Nursing Services and Administrator.
2. Assists in maintaining a clean safe environment for residents, detecting and correcting situations that have a high probability of causing accidents or injuries to residents.
3. Understands the legal implications of not carrying out physicians’ orders correctly.
4. Recognized when a situation can not be handled alone and requests help from proper personnel when necessary.

Assists with acting upon physicians’ orders and requests for restorative care.

1. Helps to evaluate residents and determine the most desirable type of treatment.
