The Licensed Clinical Social Worker is responsible for providing initial evaluations, individual, group, couples and/or family therapy. In addition will provide behavioral health consultation services to the primary care team within SHD.

Responsibilities
Essential Functions

- Assess patients as necessary through examination, chart review and patient interviews, physician interviews, and/or caregiver interviews.
- Meeting with clients to assess their histories, conditions, and needs.
- Creating treatment plans using various methods, such as behavior therapy or psychotherapeutic techniques.
- Collaborating with doctors and other health care providers.
- Counseling people and families experiencing difficult situations through individual or group therapy.
- Individual psychotherapy with children, adults, and families.
- Provide information and referral services.
- Crisis assessments/triage with appropriate interventions.
- Documentation and scheduling in electronic health record.
- Demonstrate proficient skill in evidence-based practices and theories to work with adults, children, and their families.

Education and Work Requirements
- Masters degree in social work from an accredited school or higher education
- Minimum of 3 years’ experience preferred
- Current LCSW (Texas License)
- BLS preferred.
- Experience with remote work & telehealth required
