We are pleased to announce the full opening of the Seminole Community Pharmacy and we look forward to serving our community. We accept all insurances except Med-Impact and Medicare DME, but we are working on getting those contracted as well.
Currently, we are asking that patients wishing to use Seminole Community Pharmacy please call or come by the pharmacy located in the hallway just off the 1st floor lobby of the Clinic Entrance, which is to the right of the lobby entrance.

- Insurance Card
- Name and Date of Birth for All Covered
- List of Medication Allergies for All Covered
- Current Pharmacy Information
- List of Prescriptions to Transfer from Current Pharmacy
