Restorative Nurse Aide (MHCC)

Under the general supervision of the MDS coordinator and Physical Therapist, administers restorative care which includes, transfers, walking, strength training and range of motion. Documenting all services according to the RAI manual and PT. Assists with teaching and training of all CNAs and students as needed. Keep in constant communication with charge nurse, DON, MDS coordinator and Administrator with residents needs. Complete all documentation as needed. Assist floor CNAs when needed.

Job Qualifications

Education

  • High school graduate or equivalent experience
  • Completion of a training and competency evaluation program approved by the State of Texas for Nursing Assistants

Licensure, Registry and Certifications

  • Current registry with the Nurse Aide Registry
  • BCLS
  • RNA training

Personal Job Related Skills

  • Effective oral and written communication skills.
  • Able to follow written and verbal instructions.
  • Ability to read and write in English.

Experience

Prior Work Experience

  • Entry level position
  • One (1) year of nurse aide experience in a clinical or long term care setting preferred.

Technical Training

  • Current CPR certification.
  • Proficient in operating Patient Transfer Lifts

Essential Duties and Responsibilities

Primary Responsibilities

  1. Follow restorative care plan as directed by physical therapist and physician
  2. Notify MDS coordinator of any changes needed or that were made by PT
  3. Quarterly summaries on all residents.
  4. Maintain equipment and log DME
  5. Attend care plan , IDT, QA and QAPI meetings
  6. Daily documentation on all restorative participants
  7. Update resident to know book on a weekly basis or more often if needed
  8. Obtain weekly and monthly weights
  9. Assist with all admissions and work with PT for transfer instructions
  10. Assist with education and training for all CNAs and students
  11. Assist with all pips needed with QAPI

Responsibilities when needed on the floor:

  1. Assist with carrying out the Interdisciplinary Plan of Care for residents.
  2. Assists residents with activities of daily living according to their Care Plans and MHCC Policies and Procedures.
  3. Documents residents’ care according to established MHCC P&P’s.
  4. Keeps the Charge Nurse informed regarding resident change of condition or other observations such as symptoms, reactions, or other changes in patient status.

Administers direct bedside nursing care under the supervision of the Charge Nurse which may include:

  1. Assists residents with fingernail care as scheduled and needed.
  2. Transfers residents from bed, to chair, to commode, to gurneys using gait belts and lifts as appropriate.
  3. Assists residents in bathing to include bedside sponge baths and hall showers regularly or as needed.
  4. Assists residents in oral care to include brushing teeth, brushing, soaking, and adhering dentures, and clearing oral cavity of debris after meals prior to residents resting.
  5. Assisting residents in eating by feeding, or monitoring proper airway protection methods prescribed.
  6. Maintains residents dignity and privacy at all times.
  7. Assists residents in restroom activities as needed to include changing soiled adult protection diapers, clothing and bedding, assisting to bedside commode, room commodes as needed and providing optimal perineal care as needed according to the residents’ care plans to reduce potential for infection taking care to preserve resident privacy and dignity.
  8. Ensures residents are in safe environment to include lowering beds and clearing furniture path as needed.

Provides assistance and performs support duties as needed.

  1. Prepares, maintains, and submits records and reports required by the Charge Nurse, Director of Nursing Services and Administrator.
  2. Assists in maintaining a clean safe environment for residents, detecting and correcting situations that have a high probability of causing accidents or injuries to residents.
  3. Understands the legal implications of not carrying out physicians’ orders correctly.
  4. Recognized when a situation can not be handled alone and requests help from proper personnel when necessary.

Assists with acting upon physicians’ orders and requests for restorative care.

  1. Helps to evaluate residents and determine the most desirable type of treatment.
  2. Assists with the performance of selected rehabilitative and therapeutic activities and exercises.
  3. Prepares residents for treatments.
  4. Transports residents to other departments as requested.
