Serves as the Primary Health Care Provider for residents on behalf of Seminole Hospital District healthcare providers.

Job Qualifications

Education

  • Graduate of an accredited School of Professional Nursing
  • Graduate of Nurse Practitioner program with an emphasis in Geriatric Medicine

Licensure, Registry and Certifications

  • Advanced Practice Registered Nurse ANCC Board Certified in good standing
  • BCLS
  • ACLS

Experience

Prior Work Experience & Technical Training

  • Two (2) years experience as a professional RN (preferred)
  • One (1) year Nurse Practitioner experience
  • Current CPR certification

Primary Responsibilities

  1. Assesses residents as medically necessary through physical examination, chart review, and patient, caregiver interviews.
  2. Orders and interprets laboratory, x-ray, CT, Ultrasound, and MRI, and EKG’s for continuing care of residents.
  3. Formulates diagnoses of resident conditions based on clinical exam, testing, and input from caregivers.
  4. Prescribes medications according to patient condition(s) and comorbidities including schedule 3-5 narcotics.
  5. Monitors specific medications therapeutic effects with labs to include coumadin, dilantin, digoxin, lithium, phenobarbitol, depakote, etc.
  6. Manages the treatment and evaluation of chronic health conditions such as diabetes, CHF, COPD.
  7. Communicates with family members regarding patient condition as needed.
  8. Collaborates and seeks input from supervising physicians as needed.
  9. Collaborates with Physical Therapy, Speech Therapy, Laboratory, Radiology, and Respiratory as needed.
  10. Provides 24 hour on call care and consulting and arranges for coverage during absences.
  11. Attends MHCC staff meetings daily or as called.
  12. Attends Standards of Care Meetings weekly.
  13. Attends Care Plan Meetings Weekly.
  14. Attends IDT Meetings every two weeks when residents of the facility are on Hospice.
  15. Attends Qualtiy Assurance Meetings every 3 months.
  16. Attends Leadership Meetings Monthly.
