Patient Rights and Responsibilities

You have the right to:

  • Considerate and respectful care with respect for your personal dignity.
  • Care provided in a safe setting, free from abuse, neglect or harassment.
  • Personal privacy and the confidentiality of your medical information.
  • Have a person of your choice present during your stay, decide who can visit you, and be informed if family or guest visitation must be restricted.
  • Have a language interpreter to assist you with conversations about your health throughout your visit free of charge.
  • Information about your condition, treatment options and test results.
  • Treatment for pain and suffering.
  • Accept medical care or to refuse treatment or research, and to be informed of the medical consequences of such refusal.
  • Timely access to your medical record.
  • Complete advance directives such as living well or medical power of attorney. These forms can be obtained from your nurse, chaplain, social worker or physician. We will still care for you if you do not have an advance directive.
  • Discharge planning evaluation to be sure your healthcare needs are met after you leave the hospital.
  • Provide feedback to us, both positive and negative.
    • You can provide feedback to the clinical manager for the unit or facility involved
    • You can also contact hospital’s Patient Advocate at: 833-950-0009. We will investigate your complaint through our formal complaint process and we will give you a response.
  • Take your complaint to the Texas Department of State Health Services at 888-973-0022 (toll free number) or by mail at 1100 W 49th Street, Austin, TX 78756-3199; the Medicare Hotline at 800- 633- 4227; or the Texas Medicare Quality Improvement Organization (KEPRO) at 888-315-0636.

You have the responsibility to:

  • Give your healthcare team accurate and complete information about your condition.
  • Be an active partner in your care.
  • Ask your doctor or other healthcare professional if you have questions or concerns about your care. If you don’t understand, ask again.
  • Be considerate of other patients and visitors at all times.
  • Follow hospital rules and regulations as they apply to patient care and conduct.
